Before starting the application please save your medical records, drivers license(Back and front) and Selfie on your computer, phone or tablet ready to be uploaded when prompted. Please DO NOT start the application process if your medical records (or letter from your doctor stating your diagnosis) are not available because the application will be deleted automatically.

Alternatively, BEFORE starting the application. You can email your records to k.sullivan@fpdoctors.com or have your doctor's office fax your records to 202-207-3732

After payment dont close your browser because you will be redirected to a registration form.
